We would like to thank you for choosing Dr. Cohl to be part of your health care team. The process of becoming a new patient can be quite comprehensive. It requires a commitment of time on your part. It can take at least an hour, sometimes longer, to complete the medical history form so set aside a sufficient amount of time in order to fully complete it.
In order to schedule your initial visit, Dr. Cohl asks that you please request a Comprehensive Health Questionnaire by filling out the form below. Dr. Cohl will have the Comprehensive Health Questionnaire sent you via e-mail. The forms are completely secure. Please have them completed 2-3 days prior to your first visit. This form is interactive, so your able to fill it out via your computer.
Dr. Cohl would also like you to request any records from previous doctors and specialists that you believe would be of value. You can either bring them with you or upload them on a secure server.
Once Dr. Cohl gets your questionnaire, he'll do a comprehensive review of all the materials before your initial visit so that he may be prepared to discuss further evaluation and his treatment methods with you.
On the day of your visit, Dr. Cohl will also look at any supplements/vitamins that you’re already taking. He will make any adjustments if needed.
